  • Gender (Male, Female, or Transgender)

  • Details of submitted documents
  • Introducer details / HoF details

  • Write the exact Name or any details as it is printed on the document submitted by you.
  • Parents have to submit their Aadhaar details if they are applying for their child’s Aadhaar below 5 years of age.
  • It is compulsory to provide your Email Address and Mobile Number in the Aadhaar Enrolment form because all the updates related to your Aadhaar will be shared on your registered email ID and mobile number.
  • The mobile number you write in the Aadhaar Enrolment form will automatically be linked to your Aadhaar Card.
  • You are allowed to rectify the errors and make corrections within 96 hours of enrolment and it will be free of cost.
  • If you want to make any rectifications in the future then you will need the same form to do so.
